VERIZON'S DEPENDENT CARE REMBURSEMT FUND REINSTATED EFFECTIVE WEEK ENDING 9/4/2010
Good News!

Verizon’s Dependent Care Reimbursement Fund has been reinstated effective week ending September 4th 2010. Requests for reimbursement of September’s dependent care expenses must be post marked no later than October 8, 2010.

What’s new….. Changes?
Effective with September’s re-instatement employee social security numbers are no longer required. Instead all employees are required to indicate their “employee ID” on each monthly request for reimbursement and on any new enrollment applications.

Who has to re-enroll?

Employees who were participants and receiving benefits up until the fund was suspension last May 2010 DO NOT HAVE TO RE-ENROLL. ..

Unless

1- Employees who changed childcare providers must complete and forward a new application indicating the new childcare information. Employees and providers are required to complete and sign a new enrollment application and new certification form. CWA Local 1105 members may forward their applications to Beatrice Zapata, CWA Local 1105 3223 E. Tremont Avenue Bronx New York 10461 or to the company Fund Administrator Beverly Steele at 240 E. 38th Street, flr. 15 New York N.Y. 10016. Former enrollees who have made changes must forward their new information/applications prior to submission of September’s request for reimbursement.

2- Members who wish to change the dependent formerly enrolled in the fund to a different dependent must also complete and forward a new application and certification form to the address indicated in above in paragraph 1.

CWA 1105 Members only:
Once you have received your monitoring device complete Verizon’s Pendant Program Enrollment Application. Attach a copy of your welcome letter, service contract or proof of purchase/rental and forward Verizon’s enrollment application to CWA Local 1105 c/o Beatrice Zapata at 3223 E. Tremont Avenue Bronx, New York 10461. You will be notified via e-mail or regular U.S. Mail concerning your acceptance to the program.

All other CWA members should forward their applications directly to the Fund Administrator at NY/NE Regional Work & Family Committee c/o fund Administrator 240 E. 38th Street Floor 15 New York, New York 10016.

Employees will be reimbursed on a quarter basis beginning April 2010. Reimbursements will be made directly to employees with their paychecks. A maximum of amount of $40.00 monthly is allowed. Employees are eligible for reimbursement ONLY AFTER they have incurred and paid the charges. Employees must be enrolled in Verizon’s Pendant Reimbursement Program in order to be reimbursed. Verizon will NOT reimburse any vendor directly.
