EMERGENCY RESPONSE USA offers CWA Member Discounted Rate on Pendant
CWA members who choose to order a monitoring device from EMERGENCY RESPONSE USA (formally SeniorCare911) will receive discounted rate of $25.00 monthly as opposed to the non-member regularly billed service charge of $29.95 monthly. Be sure to enter promotion code CWA 1105 to receive your discount.

CWA 1105 Members only:
Once you have received your monitoring device complete Verizon's Pendant Program Enrollment Application. Attach a copy of your welcome letter, service contract or proof of purchase/rental and forward Verizon's enrollment application to Verizon, c/o Beverly Steele. All CWA members should forward their applications directly to the Fund Administrator Beverly Steele at NY/NE Regional Work & Family Committee c/o fund Administrator 120 Hicksville Road, Room 200-A, Massapequa N.Y. 11758 .

Employees are reimbursed on a quarter basis. Reimbursements will be made directly to employees with their paychecks. A maximum of amount of $40.00 monthly is allowed. Employees are eligible for reimbursement ONLY AFTER they have incurred and paid the charges. Employees must be enrolled in Verizon's Pendant Reimbursement Program in order to be reimbursed. Verizon will NOT reimburse any vendor directly.
